Terms of Service
Seraphim OS provides healthcare practice workflow software, including patient intake, document capture, scheduling coordination, and administrative workflow support. Use of a practice deployment is subject to the applicable practice relationship, deployment configuration, and signed service terms.
Acceptable use
- Do not submit emergency medical requests through Seraphim OS forms.
- Do not misuse, probe, or attempt unauthorized access to Seraphim systems.
- Use patient-facing workflows only for the intended practice and care context.
Privacy Policy
Seraphim collects business inquiry information from the landing page and practice workflow information from customer deployments. Practice workflow data can include patient information, documents, consent records, and operational metadata needed to support the workflow.
Landing page inquiries
The Seraphim OS landing page inquiry form is for business contacts only. Do not send patient information, urgent care requests, or medical details through this form.
Logs and analytics
Production systems should avoid raw PHI in application logs. Operational logs are used for security, reliability, audit, and troubleshooting.
HIPAA Notice
When Seraphim handles protected health information for a covered entity or another business associate, it should do so under the applicable Business Associate Agreement and deployment controls. Each practice remains responsible for its own patient notices and obligations unless otherwise agreed in writing.
SMS Consent
Business inquiry consent is separate from patient SMS consent. Patient messaging consent, when enabled, must be collected in the practice workflow with clear opt-in language, STOP/HELP instructions, message frequency, and rate disclosures.
